Service Description
This comprehensive clinical evaluation is required to determine medical eligibility for our Semaglutide or Tirzepatide protocols. Our medical team will review your health history and metabolic goals to establish a safe and effective treatment plan. Fee: The $100 evaluation fee covers your physician-led GFE and clinical intake. Upon medical clearance, $50 of this fee will be applied as a credit toward your first month of therapy. Mobile Service: All protocols include an in-home visit from our nursing team for your initial injection and hands-on training. Serving the Tampa Bay Area.
Cancellation Policy
Appointment & Evaluation Fee: The $100 fee covers your mandatory Medical Evaluation (GFE) and clinical intake. This fee is non-refundable once booked, as it covers the physician’s time and the administrative costs required to review your medical history. The Credit: Upon medical clearance and enrollment in a treatment plan, $50 of this evaluation fee will be applied as a credit toward your first month of therapy. Rescheduling & No-Shows: We require at least 24 hours' notice to reschedule. Cancellations within 24 hours or "no-shows" will forfeit the $100 fee. A new evaluation fee will be required to restart the process. Medical Eligibility: If you are deemed medically ineligible for GLP-1 therapy, the $100 fee remains non-refundable as it covers the professional medical evaluation performed by our providers. The Credit: Upon medical clearance, this $50 payment will be applied as a full credit toward your first month of GLP-1 therapy (Semaglutide or Tirzepatide). Rescheduling: We require at least 12 hours' notice for rescheduling. "No-shows" or cancellations within 24 hours of the appointment time will forfeit the $50 fee, and a new evaluation fee will be required to restart the process. Medical Eligibility: In the event you are deemed medically ineligible for GLP-1 therapy by our providers, the $50 fee remains non-refundable as it covers the professional medical review performed.
